We need a written copy of your current, valid eyeglass prescription.  You may fax it to Dr. Bill Vietti at 620-431-6959 -- or you may mail it to Dr. Bill Vietti, P.O. Box 307, Chanute, KS 66720  We also need your email address so we can notify you that we have received the valid prescription.
